Student Organization Room Reservation Request

This Student Organization Room Reservation Request form is only to be utilized by Student Organizations.

Please note the following:
  • An e-mail response will be sent only to the contact person requesting the facility. Follow-up information may be required before a facility is approved for usage.
  • All room requests take at least three working days to process since they must be submitted to the proper campus official or facility for approval.
  • Requests will only be processed during regular Student Activities Office hours which are Monday - Thursday, 10:00 am - 8:00 pm
  • Requests received after 5:00 pm on Thursdays will not receive a response via e-mail to the contact person until Tuesday afternoon of the following week.
  • No requests for rooms will be honored during the regular SGA meeting times which are on the first and third Thursdays of each month from 1:00 pm to 2:00 pm.
  • Requests for the Athletic Recreation Center must be approved by the Director of Campus Recreation and an on-line form at http://www.usip.edu/arc/facilitiesrequest.aspx must be completed before final approval.
  • Requests for Wilson Student Center must be submitted at least 10 days prior to the event and after approval; a form must be completed in the Student Life Office in Whitecar Hall, First Floor at least one week before the event.
  • After receiving approval of a special event in Wilson Dining Hall, the Athletic Recreation Center and/or Alumni/Rosenberger Auditorium (old gym), an organizational member must meet with the Student Activities Administrator for set-up, insurance and security purposes.  Advisors must be present in the above locations the entire time of the event/function.
  • Completion of this form does not guarantee the room or facility requested.
  • Please complete this form entirely. Failure to do so may delay approval of facility.
